What Should Be Reported
When to Speak Up
Fraud or Financial Misconduct
Billing fraud, falsified records, theft, misuse of funds or patient property.
Abuse, Neglect, or Exploitation
Physical, emotional, or financial abuse of patients. Neglect of care duties.
Safety Concerns
Unsafe practices, environmental hazards, medication errors, impaired staff on duty.
HIPAA Violations
Unauthorized disclosure of patient information, unsecured records, privacy breaches.
Regulatory Violations
Violations of Ohio law, OSHA standards, or NobleCare policies and procedures.
Retaliation
Any adverse action taken against someone for reporting a concern in good faith.
How to Report
Four Reporting Channels
Direct Report
Speak directly with your supervisor or the Administrator.
Written Report
Submit a written complaint to the NobleCare office.
Mark envelope: "COMPLIANCE -- CONFIDENTIAL"
Anonymous Report
Use the form below or email our compliance address. You do not have to provide your name.
External Agencies
You may contact external agencies at any time -- you do not need to report internally first.

Non-Retaliation Policy
NobleCare Home Health strictly prohibits retaliation against any person who reports a concern in good faith, participates in an investigation, or cooperates with an external agency. Retaliation is itself a serious violation and will result in disciplinary action up to and including termination.
