Reportable Changes -- Call Your Supervisor
- Fall (with or without injury)
- New or worsening pain
- Changes in mental status or confusion
- Fever > 100.4 F
- Skin breakdown, new wounds, or pressure areas
- Refusal of care or medications
- Signs of abuse, neglect, or exploitation
- Medication errors
- Equipment malfunction
- Hospitalization or ER visit
- Significant change in appetite or weight
- Unsafe home environment
