Reportable Changes -- Call Your Supervisor

  1. Fall (with or without injury)
  2. New or worsening pain
  3. Changes in mental status or confusion
  4. Fever > 100.4 F
  5. Skin breakdown, new wounds, or pressure areas
  6. Refusal of care or medications
  7. Signs of abuse, neglect, or exploitation
  8. Medication errors
  9. Equipment malfunction
  10. Hospitalization or ER visit
  11. Significant change in appetite or weight
  12. Unsafe home environment
