Participate in Your Care Plan
You have the right to be involved in developing, reviewing, and changing your plan of care, and to be informed of any changes before they take effect.
Informed About Your Services
You have the right to be informed in advance about the services to be provided, the expected outcomes, and any risks involved.
Refuse Care
You have the right to refuse any service, treatment, or caregiver at any time, without fear of reprisal or discrimination. We will explain any potential consequences, but the decision is always yours.
Freedom from Abuse and Exploitation
You have the right to be free from all forms of abuse, neglect, exploitation, and discrimination.
Voice Complaints Without Fear
You have the right to voice complaints and grievances about your care or treatment without fear of retaliation, discrimination, or interruption of services.
Access Your Records
You have the right to access and obtain copies of your own clinical records upon written request.

You have the right to be informed of the agency's charges, payment policies, and billing procedures before services begin.
Choose Your Provider
You have the right to choose your home health care provider and to be informed of other providers available in your area.

How to File a Grievance
If you are unsatisfied with any aspect of your care, or if you believe your rights have been violated, you have several options:
